Blank Forms
UPDATED JUNE 14, 2007 WITH 2007-2008 GRANT APPLICATION!!
For best results, save the Grant Application form to your
computer before opening it, filling it out, and/or printing it. To save it to
your computer...
Windows:
Right-click the link for
your chosen format, then choose "Save Target As..." (Internet
Explorer) or "Save Link As..." (Netscape).
MacIntosh:
Click and hold the link for your
chosen format, then choose "Save this link as...".
There are three formats available online, with some important changes
this year. If you have Acrobat
Reader version 7 or later, you can fill out the pdf application and save it with your
information. This version
has additional features that simplify completion of the form, and is the
recommended format since it is supported well by Windows and MacIntosh. If you have Acrobat Reader 6 or earlier,
you can install the latest version (free) by clicking the following icon:
- Adobe Portable
Document Format ".pdf". If you have access to Adobe Acrobat
reader 7 or later, you can view, fill out, save, and print the
application. If you have Adobe Reader 7.0, you may receive messages
indicating that a component is missing. If you cancel the messages,
you should be able to complete and save the application without any problems.2007-08 Grant Application in Adobe pdf format
- Microsoft Word
".doc" (compatible with Word 97 and later). If you have access
to Microsoft Word, you can fill out the application in Word, then print
it out. 2007-08 Grant Application in Microsoft Word format.
- Rich Text Format
".rtf". If you don't have Microsoft Word and don't want to
upgrade to Adobe Reader 7 or 8, but would like to complete the
application electronically, you may be able to use this format. 2007-08 Grant Application in Rich Text Format
Helpful Hints:
- The application form
is designed to allow entry
only into the designated fields. To move from field to field, use the
"Tab" key to move to the next field, and Shift-Tab to move to
the previous field. (With the rtf format, this may vary depending on
the word processing program you are using.)
- Clicking on a check
box will change it from unchecked to checked, or checked to unchecked.
- If you are using the Word or
Adobe Reader version,
pressing Tab to move from entry to entry will automatically update the
Sub-Total, Sales Tax, and Total. The Sales Tax % can be changed if 7.75% doesn't
apply to your grant.
